My company goes through a certain financial group for the short term disability that they had to file the claim through. As for the FMLA, I signed a customized form that my company made for themselves. Every company is different so I don't think you can find a universal form for either of these two, but hopefully this info on these forms could help.

What is involved in pregnancy, like any changes of sleep, appetite, or mood?
Q. My girlfriend and I aren't planning on kids yet. But in the future what would i expect from her pregnancy, will any of her dailylife rountines differentiate from their regular basis? Because I haven't necessarily have had the opportunity to hear about these things relating pregnancy.Because any of those changes that occur I WILL support her and be there to help her along the way. A. There is no way to know for sure. Pregnancy and how it effects a woman's body is very much an individual thing.
But there is a good site that does give helpful info. I track my pregnancy on there weekly. It you look at the top,you can look at how a pregnancy progresses from when a woman ovulates all the way until delivery. And also talks about things that she could go through physically such as symptoms etc.
There is also videos that you can watch about labor and birth,and even caring for a newborn.

What do you think could be done to help prevent teen pregnancy?
Q. Do they need more info on it? Do they need one of those crying baby dolls? Does every young girl need to put on the birth control shot at puberty?
A. If it was not unethical, I would say to give them each a foster baby for a weekend. I know it is completely unrealistic, but a weekend being in complete responsibility for another human life would certain take the 'glamour' aspect out of the teen pregnancy thing. I also think that there needs to be more parental involvement...but that is also not something I can control. When we have these young girls growing up seeing their parents behave in a certain way, they are more likely to behave the same way (which will get them pregnant). I work with addicts and it is amazing the similarities in the stories of the teen mothers. They basically grew up thinking the same thing about love and affection. And it leads to little babies. It would be great to have school-sponsored self-esteem classes. I think that would help out some. But I do not say that they should be required to get a birth control shot, that is an infringement on personal rights.

A. An irregular period is actually an irregular ovulation. A woman goes through phases in her cycle, and the time from the ovulation to the next period is nearly always the same each month for every individual woman. I found this out when I was charting my cycles when trying to conceive. I have VERY irregular periods (anywhere from 30 to 60 days), but once I ovulated it was ALWAYS 13 days until my next period. That's how I determined when to do a test.
If you aren't charting, the way to do a test accurately while not knowing when to expect your period is to test 2-3 weeks after you have sex. Are you trying to conceive? Irregular periods might make timing a little more tricky, but itself does not make you infertile. You could also try using an ovulation predictor kit if you are not comfortable with charting, or a combination of both. If you want more info about either, just google them and you should find info about both. Good luck!
